We are contracted with and accept major private insurance plans listed below.  However, insurance contracts are subject to change without notice.  It is the patient’s responsibility to seek care from a participating provider and get highest level of benefits from their managed care plan. Please call your insurance to get more information about your medical and behavioral health benefits.  We also offer discounted rates for private-pay patients.  If we currently do not accept your insurance, we will be happy to provide you services at private-pay / cash-pay rates.
